Summary
During well abandonment at MI A-5, a crane fast line parted as the operator lowered a power swivel without sufficient slack, dropping the ball onto the deck. Nobody was injured. The investigation identified absent weight indicators as a contributing factor and documented reattachment testing and inspection before return to service.
