Summary
BSEE investigates a facial injury during Hilman roller replacement on the Ocean Blackhawk drillship. Miscommunication led to hydraulic jack depressurisation; the falling jack sheared an adjacent jack’s fitting, releasing a hydraulic line that struck the mechanic. Findings address unsecured equipment, missing rigid support, body positioning and inadequate supervision.
