Summary
BSEE investigates a rigger’s index-finger fracture during riser-insert lifting aboard the Globetrotter I drillship. His finger became trapped between an air-hoist cable and sheave. Findings address hand placement, the supervisor’s failure to request a crane, failure to stop unsafe work, and inconsistent critical-lift classification across work-planning documents.
