Summary
BSEE investigates a roustabout’s finger laceration and fracture during basket repositioning on the Parker 77B barge rig. A double studded adapter shifted, trapping his hand against the basket rim. Findings address incomplete job planning, procedural failures, communication and unsecured contents; the findings and violation narrative differ on air-hoist use.
