Summary
BSEE investigates a fatal drowning and serious injury during galley floor repairs on Platform Gilda. Two workers fell approximately 80 feet into the ocean when a non-load-bearing moisture barrier collapsed. Findings address material misidentification, inadequate task hazard analysis, unrestricted access and failure to stop work, with recommendations covering planning and water rescue.
