Summary
Investigation of a tubing hanger pin ejection during temporary well abandonment, causing a fractured shin. A defective fitting allowed hydraulic fluid into a wellhead void, compressing trapped air or gas. Findings address unrelieved pressure, improper pin removal, unavailable tools, supervisory training oversight and incomplete job safety analysis.
