Summary
Investigation of a synthetic-based mud release during transfer from Master Everett to West Sirius. A leaking mud pit dump valve and an incorrectly fitted master valve operator restricted closure, releasing 3,123 barrels into Gulf waters. Possible solids accumulation was identified for the pit valve; no regional recommendations were made.
