Summary
Investigation of a pipeline-pump fire on Ship Shoal 80 Platform A during production restart. A failed packing gland sprayed oil onto engine exhaust. The report attributes packing failure to cavitation and severe vibration while both pumps ran simultaneously, and records absent shielding, helicopter evacuation and vessel-assisted firefighting.
