Summary
An investigation describes a facial injury during well hot bolting while piping pressure was being equalised. Pressure of up to 5000 psi escaped at a flange after multiple bolts were removed. Findings identify misunderstanding of the task, an unsigned job safety analysis, lone working without control-room notification and failure to de-energise equipment.
