Summary
BSEE investigates two injuries during completion-line flushing aboard the Deepwater Proteus drillship at Mississippi Canyon 899. Disconnecting a six-inch camlock connection released trapped pressure and propelled a hose into workers. Findings address premature valve closure, inadequate whip-check placement, omitted chemical-protective clothing and gloves, and failures to implement the job safety analysis.
