Summary
Investigation of a platform crane boom failure during well abandonment at VR 340A. Findings identify worn pin-connection bores, an incorrect load chart and use of static rather than dynamic limits. Corrosion, weight-indicator adjustment and unaccounted load-block weights are also discussed. No personnel were injured; inspection and procedural improvements are recommended.
