Summary
Investigation of an electrical flash during relocation of a transfer-pump connection from a 20 amp to a 30 amp receptacle. A derrick hand sustained burns when energised components exploded. Findings identify failure to switch off the breaker, absent job safety analysis, inadequate training and failure to follow glove and fire-resistant clothing requirements.
