Summary
During a transfer of the second end of a 4" (10 cm) production flexible, the production flexible began rotating due to residual torsion that had built up within the line. A 16.5te chain attached to the pull-in head also began spinning. Two nearby crew members who were observing the operation were almost struck by the spinning chain.
