Summary
This investigation examines two gas leaks during Statfjord B’s restart after a turnaround on 23 May 2022. External corrosion compromised carbon-steel blowdown lines. Findings address deferred surface maintenance, inadequate visual inspection and pipe identification, outdated risk-based inspection assessments, startup planning and emergency response, including missed deluge activation.
