Summary
Investigation of a derrickman’s head injury during plug and abandonment work on Rowan Juneau. A casing offcut passed through an uncovered gap, lodged below the rig floor and subsequently fell. Findings address scrap accountability, incomplete opening coverage, omissions in job safety analyses, an unclear safety-watch role and ineffective supervision.
