Summary
An investigation examines mezzanine-deck installation at the MC 736 A platform. The load jammed against pressurised gas-lift pipework, and attempts to free it damaged piping and an isolation valve. Findings identify departures from engineered lifting arrangements, inadequate hazard assessment and delayed stop-work intervention. No injuries or hydrocarbon releases were reported.
