Summary
An MMS investigation examines a roustabout’s fall of approximately 30 feet while attaching lifting straps to stacked risers. It discusses excessive stack height, unavailable suitable anchoring arrangements and departures from the written THINK plan. Wet surfaces may have contributed. The worker sustained chest, back and shoulder injuries and was evacuated for treatment.
