Summary
BSEE investigates a deckhand’s rib fractures while retrieving a job safety analysis from a personnel basket aboard M/V Mr. Lloyd. The basket was lifted while the operator’s view was obstructed. Findings examine departure from agreed hand signalling, an unidentified radio command on an open marine channel and failure to revise the lift plan.
