Summary

Investigation of a flash fire that burnt a contractor opening a master panel on an unmanned satellite platform. A failed regulator O-ring released gas, most likely ignited by a lit cigarette. Findings address smoking restrictions, absent job safety analysis, failure to stop work and maintenance work extending beyond the contractor’s scope.
