Summary
This investigation examines a technician’s bicep tendon tear while unloading a crane-suspended basket at High Island A 474 A. Congestion prevented landing the basket directly on the deck. Findings identify omissions in the behavioural job safety analysis concerning lift planning, stop-work authority, mechanical lifting, straining and overextending.
