Summary
A roustabout aboard the Ocean Blackhornet drillship tripped over an air hose while carrying a chiksan swing during coiled-tubing unit setup. The swing struck his right hand, requiring evacuation and surgery. BSEE identified inadequate hazard recognition and job safety analysis, and issued noncompliance findings concerning supervision.
