Summary
BSEE investigates a blackout and drift-off aboard Noble Stanley Lafosse during accommodation fire-damper testing. Inadvertent operation of emergency fuel shut-off valves preceded loss of station keeping, emergency disconnection and brine discharge. Findings address missing task risk assessment, unused stop-work authority, inadequate assistance and unprotected valve controls.
