Summary
An investigation examines an operator’s eye injury while checking an orifice plate on a production platform. Loosening clamp bolts released natural gas and debris at approximately 915 PSIG. Findings identify an unclosed equaliser valve and a blocked blowdown valve that gave a false impression of depressurisation. Medical treatment included irrigation and surgery.
