Summary
An investigation records a hydraulic hose failure during platform structural removal using a prime shear. Following repair of a small leak without pollution, increased hydraulic pressure caused the line to fail, releasing 55 gallons of hydraulic oil into Gulf waters. Abrasion was identified as the cause; the shear was removing underwater debris.
