Summary
Investigation of a platform chemical injection incident in which a construction worker attempted to kink leaking tubing by hand. A second burst sprayed chemical into both eyes, causing retinal burns. Findings identify abrasion-susceptible tubing with a working-pressure limit below the pump’s maximum discharge, and delayed regulatory injury notification.
