Summary

An investigation examines a contract lead operator’s fractured ribs during gas-generator starter troubleshooting. After a union was disconnected, opening the Amot valve released pressure that made the piping rotate and strike him. Findings identify omitted job safety analysis, failure to follow hazardous-energy controls and an unused pressure-checking arrangement.
