Summary
A gas production platform flash fire followed a fatigue crack in threaded glycol return pipework. Released gas and condensate ignited on an uninsulated reboiler stack. Vibration was identified as the probable fatigue cause. Recommendations address vibration damping, periodic pipework inspections and insulation or double-wall exhaust stacks at potential ignition sources.
