Summary
Investigation of a well toppled during Hurricane Isaac at MP 116 beneath Ensco 81. Sonar identified the well resting on the seabed; failure appeared near a weld above the mud line. Findings discuss hurricane preparations, absent ultrasonic weld inspection and possible welding and drive-pipe design deficiencies. No recommendations were made.
