Summary
BSEE investigates a tool-string ejection during temporary abandonment of the E-4 well at Ship Shoal 224 E. Firing an electric-line casing cutter released trapped annular pressure beneath a subsea tieback wellhead, damaging deck grating without reported injuries. Findings address unrecognised pressure communication restrictions, absent wireline pressure control and planned procedural changes.
