Summary
BSEE investigates a contractor’s thumb laceration during depressurisation after a Flexi-Coil hose pressure test on West Delta 109 A. Rapid pressure release caused the unsecured hose to whip and a ball valve to break against the reel. Findings address valve operation, hose securing and glove selection; fog constrained evacuation to vessel transport.
