Summary
An employee checking a suspected vent-line valve leak on a test separator lost his footing while stepping away from low piping and struck a blind flange, fracturing two ribs. The investigation identified failure to observe surrounding trip hazards as the probable cause and made no recommendations to the Office of Safety Management.
