Summary
Investigation of an electric-line wire-rope failure during temporary well abandonment at Apache WC 102-2. A bridge-plug assembly fell and injured a worker’s foot. Laboratory examinations confirmed corrosion-related shear/overload fractures. Findings address departure from the approved workstring method, incomplete job safety analysis and missing maintenance records.
