Summary
BSEE investigates a floorhand’s hand injury during well decompletion on the Auger platform. A released split wear bushing bounced from a rubber rotary mat and struck his hand. Findings identify improper hand placement, task inexperience, inadequate hazard analysis, unused support lines and hands-free tools, and congested workspace. Medical evacuation and restricted work followed.
