Summary
An investigation of a platform lifting incident in which a nylon sling melted following exposure to turbine-generator exhaust heat. A flowline riser fell approximately 20–25 feet, trapping a rigger between an air-hoist cable and handrailing. The report describes rescue and medical evacuation, examines sling selection and recommends restrictions on nylon-sling use.
