Summary
Investigation of a man-riding basket damaged during pipe-racker maintenance. At approximately 110 feet above the rig floor, the basket appeared to snag and tilted; its hanging arm separated after lowering. Findings address radio communication, wind assessment, supervision, training, rescue planning and inadequate independent secondary fall protection.
