Summary
Investigation of a crane incident in which tubing slipped and fell 2–3 feet to the deck, with an employee reporting being struck and sustaining a foot abrasion. Winch teardown found normal clutch wear without component failure evidence. More than 50 subsequent brake tests did not reproduce the slipping.
