Summary
An investigation describes a drilling-rig mouse-hole section falling onto a production platform, damaging well equipment. Leakage of between 4–5 gallons of hydraulic fluid led to down-hole safety valves closing and production shutting in. Extended use and over-torquing could have caused cracking and fatigue; periodic inspection for wear and damage is suggested.
