Summary
Investigation of a blind cargo-box lift in which a worker’s leg became entangled in a tagline, pulling him overboard before he fell 106 feet into the Gulf of Mexico. Findings address personnel clearance, tagline guidance, rigger re-verification, missing permits and lift plans, and omitted pre-job hazard discussion.
