Summary
Investigation of a glycol reboiler fire during restart of West Delta 152-A after an extended shutdown. BSEE identified overpressure releasing glycol and contaminants, with the stack the most likely ignition source. Findings address unattended startup, inadequate procedures, training and maintenance; corrective measures include stack insulation and competency assessments. No injuries or pollution were reported.
