Summary
An investigation examines equipment damage during BOP pressure testing on Nabors 17 at West Delta 73. A missing bull plug allowed pressure beneath a Greens cup tester to propel the test assembly upwards. Findings identify training, job safety analysis and instruction deficiencies, and unused stop-work authority. No injuries occurred.
