Summary
Investigation of a platform crane collapse during casing removal for plug and abandonment work. Possible swing gear bolt failure from exceeding load capacity was identified as the probable cause. Findings address missed ring-bearing inspections, operator experience, unavailable load information and missing JSA documentation. No injuries or pollution were reported.
