Summary
BSEE investigated drill-line unspooling during derrick extension on Nabors MODS 140. Insufficient line on the drawworks drum allowed the travelling block to settle on the rotary table and the fast line to fall. No injuries or damage were reported. Findings identified omissions in the job safety analysis and rig-up procedures.
