Summary
A pry bar slipped during removal of tools from an elevated work platform at Horn Mountain, deflected off a ladder cage and struck a worker nearby. The worker lost three teeth and sustained a lip injury requiring 25–30 stitches. The report cites poor planning and provides no recommendations.
