Summary
Investigates a hand injury during crane-assisted handling of a mechanical casing cutter on South Pass 49 C platform. A grease-locked casing jack bowl separated as the cutter changed orientation, trapping an operator’s hand. Findings examine unsecured components, ineffective implementation of a no-hands policy, missing handling aids and inadequate task-specific hazard assessment.
