Summary
An investigation examines a foot injury during removal of a water-skimmer manway hatch. Failure of its support allowed the hatch to fall. Findings address corrosion, washer positioning, absent inspection and inadequate task-specific hazard analysis. Separate lockout/tagout deficiencies were identified in photographs, with potential hydrocarbon ingress during confined-space entry.
