Summary
BSEE investigates a riser adapter failure during disconnection following temporary well abandonment. Reduced riser tension allowed the BOP stack to tilt and subsequently fall to the seabed. Finite element analysis supported the failure explanation. Findings identify inadequate procedures and undefined safe operating parameters; the plugged well caused no attributed pollution.
