Summary
BSEE investigates a dropped drill-pipe stand during well abandonment on Thunderhorse PDQ. Elevators opened before the pipe racking system secured the stand. A floor hand was injured while escaping. Findings identify failure to follow tripping instructions and poor communication between the driller and assistant driller.
