Summary
This investigation examines an overboard synthetic-based mud discharge during casing head washing. Misaligned valves, poor communication after pit cleaning and an undocumented transfer-line connection contributed to the release. Recommendations address valve locking and permit approval, clearer task responsibilities, revised cleaning procedures, piping verification, labelling and mud-system training.
