Summary
Investigation of a well-control incident during abandonment work on 15 May 2012. Pressure below a cast iron bridge plug ejected the work string and bottom hole assembly, damaging equipment and a diesel tank. Blowout preventers sealed the wellbore; no injuries occurred.
