Summary
This investigation examines a production operator’s disappearance during monthly casing pressure checks on Eugene Island 331 B platform. The operator was never located. It analyses displaced hazardous grating, maintenance and barricading failures, contractor arrangements and lone-working procedures, distinguishing probable causes from possible causes and recommending improvements to work controls.
